Access to pediatric subspecialty care continues to be a challenge in the US.1-3 There are approximately 72.7 million children (<18 years of age) in the US (childstats.gov), and this number has been relatively stable over the past 2 decades (72.7 million in 2001 and 74.1 million in 2008). However, in recent years, the survival of children with congenital anomalies, disorders of metabolism, conditions such as cystic fibrosis, or a history of prematurity has increased.1,4 In addition, there has been an alarming increase in the prevalence of obesity as well as behavioral and mental illness among children.
