Mashup Score: 22Endoscopic Ultrasound Guided Biliary Drainage in Malignant Distal Biliary Obstruction - 8 month(s) ago
Malignant biliary obstruction (MBO) is a challenging medical problem that often negatively impacts the patient’s quality of life (QoL), postoperative complications, and survival rates. Endoscopic approaches to biliary drainage are generally performed by ERCP or, in selected cases, with a percutaneous transhepatic biliary drainage (PTBD). Recent advances in therapeutic endoscopic ultrasound (EUS) allow drainage where previous methods have failed. EUS has evolved from a purely diagnostic technique to one that allows a therapeutic approach in the event of ERCP failure in distal MBO. Moreover, the introduction of dedicated accessories and prostheses for EUS-guided transmural biliary drainage (EUS-BD) made these procedures more successful with regard to technical success, clinical outcomes and reduction of adverse events (AEs). Finally, lumen-apposing metal stents (LAMS) have improved the therapeutic role of the EUS. Subsequently, the electrocautery enhanced tip of the LAMS (EC-LAMS) allows

How can one safely and effectively reach the cecum in a long, redundant, and loopy colon? This is a question that several of us ask ourselves during colonoscopy. In this edition of Top Tips from master endoscopists, Dr Doug Rex, president of the American Society for Gastrointestinal Endoscopy, provides us with clear and precise insights for this important topic. Starting from endoscope selection and patient positioning and continuing to loop reduction, Doug, as always, does a masterful job of walking us through the steps on how to approach a patient during a difficult colonoscopy.

T he Practicing Endoscopist Posts What is the optimal way to use variable stiffness colonoscopes in difficult colonoscopies? Have you had any adverse events? Variable stiffness is a wonderful addition to colonoscopes, especially pediatric colon oscopes, which I use for most colonoscopies in the hospital where I have these scopes. At my other hospital we have a different scope brand (Fujifilm), which are also wonderful scopes. Variable stiffness is especially useful in floppy colons, elongated sigmoid

orrhage, which may necessitate prompt haemostatic intervention. Reversal of DOACs may also be required for patients in need of urgent invasive procedures. This guideline from the European Society of Anaesthesiology and Intensive Care (ESAIC) aims to provide evidence-based recommendations and suggestions on how to manage patients on DOACs undergoing urgent or emergency procedures including the treatment of DOAC-induced bleeding. DESIGN A systematic literature search was performed, examining four drug comparators (dabigatran, rivaroxaban, apixaban, edoxaban) and clinical scenarios ranging from planned to emergency surgery with the outcomes of mortality, haematoma growth and thromboembolic complications. The GRADE (Grading of Recommendations, Assessment, Development and Evaluation) methodology was used to assess the methodological quality of the included studies. Consensus on the wording of the recommendations was achieved by a Delphi process. RESULTS So far, no results from prospective r
