Disparities in Health Care in Patients With Chronic Liver... : Journal of Clinical Gastroenterology - 1 day(s) ago

tion. CLD disproportionately impacts racial, ethnic, sexual, and gender minorities. CLD is also more prevalent in populations experiencing challenging social determinants of health (SDOH) factors. Health disparities are prevalent across various forms of CLD, reflected in higher disease prevalence and severity, premature or excess adverse clinical outcomes and worse patient-reported outcomes. In this review, we highlight current epidemiology and health disparities across major forms of CLD, including metabolic dysfunction-associated steatotic liver disease (MASLD), alcohol-associated liver disease (ALD), viral hepatitis, and autoimmune liver disease. Herein we discuss challenges with respect to disparities in screening, diagnosis, noninvasive testing for risk stratification, treatment, and rates of clinical outcomes. We outline actionable interventions from a broad public health level to the individual patient level with the goal of moving towards health equity. Lastly, we highlight are

Diagnostic Yield of Video Capsule Endoscopy (VCE) in Celiac ... : Journal of Clinical Gastroenterology - 1 day(s) ago

ased on CD-specific serology and small bowel biopsy. Video capsule endocopy (VCE) is a relatively safe method that provides high-resolution imaging of the entire small intestine mucosa. Today, VCE assists in CD diagnosis in many circumstances. Methods: We systematically searched the medical literature databases up to December 31, 2023, for English-language studies on CD diagnosis by the VCE. Our inclusion criteria comprised complete articles with extractable data and focused on the VCE yield of known celiac patients. Following data extraction, a Meta-Analysis was performed using Comprehensive Meta-Analysis Software (version 4; Biostat Inc., Englewood, NJ). Results: We found 22 studies and 46 substudies published up to 31.12.2023 of VCE performed in CD and met our inclusion criteria. Together 1585 patients were studied, of whom 1253 (79.05%) were women. The average age of the patients was 51.94Β±9.98 SD. A complete small bowel investigation was achieved in 1533 (96.72%) of the patients.

Adenomatous Polyposis Coli Gene Mutations, Risk Factors,... : Journal of Clinical Gastroenterology - 1 day(s) ago

ve study included the data of 81 patients with familial adenomatous polyposis who underwent surgery between 1978 and 2021. The adenomatous polyposis coli gene mutation sites, risk factors, and long-term outcomes associated with desmoid tumors in Japanese patients with familial adenomatous polyposis after colectomy were analyzed. Results: No association was observed between the gene mutation sites and desmoid tumor development in 40 patients who underwent genetic analyses. The rate of desmoid tumor development was 30.3% in 66 patients. Multivariate analysis revealed that age below 32 years at colectomy (hazard ratio = 5.491, 95% confidence interval 1.820-16.50, P < 0.001) and familial adenomatous polyposis-related malignancies other than colorectal cancer (hazard ratio = 5.574, 95% confidence interval 2.075-14.98, P < 0.001) were independent risk factors for desmoid tumor development following colectomy. The 10-year disease-specific survival and overall survival rates for desmoid tumors

ia (SIBO) and explore the potential relationship between LBT and RGERD to identify a new treatment method for RGERD. Methods: A total of 178 patients with gastroesophageal reflux were enrolled from June 2020 to December 2022 in the Gastroenterology Department, Building 3, the First Affiliated Hospital of Kunming Medical University; these patients included 96 patients with nonrefractory GERD (NRGERD) and 82 patients with RGERD. The Gerd Q score, reflux symptom index (RSI) score, gastroscopy results, clinical symptoms, and other related indicators were collected. Statistical methods were used to analyze the gathered data. Results: The incidence of acid reflux and heartburn in patients with RGERD was significantly greater than that in patients with NRGERD (67.10% vs. 42.70%, P<0.01 and 65.00% vs. 34.40%, P<0.01). The CH4 values of patients with RGERD were significantly greater than those of patients with NRGERD at each time point, and there was a correlation between the CH4 values at 60 m
