      Accurate ascertainment of end-stage kidney disease (ESKD) in electronic health records (EHRs) data is important for much epidemiological research. This study aimed to develop and validate an algorithm using diagnosis and procedure codes to identify patients with ESKD (treated with maintenance dialysis or kidney transplantation) in EHRs data.

      Heart failure (HF) and kidney disease are intricately linked.1 Approximately 10%-30% of patients with chronic kidney disease (CKD) have HF, while ∼30%-60% of patients with HF have CKD.2 This population of patients with concurrent HF and kidney disease, which continues to grow, is at elevated risk of loss of kidney function, frequent hospitalizations, and death.3 The interplay between HF and kidney disease is complex and multifactorial. The contribution of volume overload has long been postulated as a central mechanism leading to kidney disease, both acutely and in the long term.

      Mixed acid-base disorders are frequently encountered in clinical practice and due to their complexity, diagnoses and treatment are challenging. The opposing effects of multiple disturbances can lead to blood pH levels that are near normal or separate acid-base disturbances can have an additive effect leading to more significant changes in blood and intracellular pH. A systematic evaluation of patient history, physical examination, and laboratory results (including acid-base balance, anion gap, electrolytes, and urine parameters) enables the rapid diagnosis of mixed acid-base disorders by analyzing the appropriateness of compensatory mechanisms, assessing changes in serum and urine electrolytes, and estimating urinary acid excretion.
