Concerns regarding five domains (study limitations, inconsistency, indirectness, imprecision, and publication bias) can lower confidence, and three domains (magnitude of effect, dose-response gradient, and residual confounding) under select circumstances may increase confidence in the CoE. A CoE rating should be provided for every outcome in a systematic review and every guideline recommendation.
