      Using genomic classifier scores, older men with low-risk prostate cancer were shown to harbor more aggressive disease than younger patients. This suggests that elderly patients with presumed-indolent disease may harbor a more aggressive phenotype, possibly requiring more active treatment.

      Although testicular germ cell tumors are highly responsive to platinum-based chemotherapy, our study demonstrates that regression of venous tumor thrombus after chemotherapy alone is less likely. Thus, complex multidisciplinary management including venous tumor thrombectomy at the time of postchemotherapy retroperitoneal lymph node dissection is typically necessary.

      The GRADE certainty of evidence (CoE) rating qualifies to what extent we can trust a body of evidence summarized in a systematic review. Concerns regarding five domains (study limitations, inconsistency, indirectness, imprecision, and publication bias) can lower confidence, and three domains (magnitude of effect, dose-response gradient, and residual confounding) under select circumstances may increase confidence in the CoE. A CoE rating should be provided for every outcome in a systematic review and every guideline recommendation.
