A 48-year-old man with hand-movement vision and total cataract (A) in the left eye was detected with a mushroom-shaped lesion (B, arrow) on screening ultrasonography. When magnetic resonance imaging revealed no abnormality (C), the Ocular Oncology Service was approached to rule out a ciliary body tumor. An ultrasound biomicroscopy revealed an intumescent lens (D, arrow) and a normal ciliary body, confirming the absence of a ciliary body tumor. An obliquely scanned intumescent lens can appear mushroom-shaped, mimicking a melanoma.

A 55-year-old woman presented with ocular pain after sequential bilateral cataract surgery. Her vision was 20/50, and her intraocular pressure was 40 mmHg in both eyes. There was bilateral chemosis and shallow anterior chamber with peripheral iridocorneal touch (A, B). Ultrasound B-scan revealed choroidal detachment (C), and ultrasound biomicroscopy demonstrated peripheral ciliochoroidal effusion (D). Since the presentation was bilateral, an idiosyncratic reaction was considered. She was administered hydrochlorthiazide 12.5 mg daily for high blood-pressure 1 month previously.
