Combination Cholesterol Therapy: A Lifesaver

Dear readers,

In all humility, my patients who need lipid-lowering therapy (LLT) have been getting 2 Rx for years: a statin at a low dose and ezetimibe. The results are great, the side effects are few, and the RACING trial from Korea a few years back showed plaque resolution with combination therapy. And, always, with CoQ10. Now, a major analysis predicts how many lives could be saved with the universal adoption of that approach. The American Heart Association’s Life’s Essential 8 is not just a heart tool; it works for cancer risk, too. More data shows that we know so little about HDL-C, as both low and high levels raise risk. A presentation at the ACC indicates that quality plant-based diets promote longer lives. If you drink drip coffee, use an unbleached organic filter to capture the cholesterol raising chemicals. Move more, even if for short periods. And two points on oils. A debate about oil-free vs olive oil in plant diets, an excellent review on why Omega-6 has been maligned, and why plant oils (yes, seed oils), are healthy. Let the screaming begin!

Be well,

Joel Kahn, MD, FACC

    • The largest analysis to examine the best way to lower levels of "bad" cholesterol in patients with blocked arteries shows that they should immediately be given a combination of a statin and another drug called ezetimibe, rather than statins alone. This could prevent thousands of deaths a year from heart attacks, strokes and other cardiovascular diseases.

    • A new Swedish study reveals that many common office coffee machines fail to filter out cholesterol-elevating compounds found in coffee, sometimes leaving behind even more than espresso or French press. Depending on the machine and brewing method, the amount of harmful diterpenes like cafestol and kahweol can vary widely, and frequent coffee drinkers may be unknowingly boosting their LDL cholesterol levels.

    • Clinical trial evidence shows the LA does not promote inflammation or oxidative stress. The processing required to produce seed oils involves several steps that are regulated and completed according to best practice safety limits. In conclusion, seed oils are a healthful dietary component that should be used to replace foods high in saturated fat.

    • Is olive oil a wholesome kitchen staple, a dietary landmine, or something in between? The answer depends on who you ask. Since the debut of the Forks Over Knives film in 2011, FOK has followed and shared the advice of whole-food, plant-based (WFPB) pioneers who’ve long counseled heart disease patients to steer clear of all added oils—even olive oil—because they are the world’s most calorie-dense foods, highly refined and stripped of most nutrients other than fat.
