Left atrial appendage occlusion (LAAO) is traditionally performed with transesophageal echocardiography (TEE) guidance, often necessitating general anesthesia and additional personnel (an imaging cardiologist and anesthesiologist). Intracardiac echocardiography (ICE) has emerged as an alternative imaging modality.1 With the advent of real-time three-dimensional ICE (3D ICE), operators can obtain multiplanar and volumetric imaging with digital steering under conscious sedation,2,3 (Figure 1A) potentially improving procedural workflow, and reducing staffing needs.
